Qualifications

• Graduate from an ACPE-accredited College of Pharmacy.

Responsibilities

• Dispense medications accurately and promptly.
• Resolve formulary and non-formulary questions per policies.
• Provide dosing and therapy recommendations per patient age and literature.
• Perform scheduled clinical chart reviews.
• Conduct Patient Care Unit inspections as scheduled.
• Prepare parenteral meds using sterile technique.
• Monitor inventory and alert supervisor about shortages.
• Document pharmacotherapy interventions to improve therapy.
• Report adverse drug events and medication errors.
• Take responsibility for the operation of the Pharmacy area and staff.
• Maintain pharmacology knowledge through publications, CE, and conferences.
