Qualifications

• Graduate from an ACPE-accredited College of Pharmacy.
• Licensed Pharmacist in good standing.
• Minimum 1 year of experience as a licensed pharmacist.

Responsibilities

• Dispenses medications accurately and in a timely fashion.
• Resolves questions regarding formulary and non-formulary medications, following policies and procedures.
• Provides dosing and therapeutic recommendations based on patient age and kinetics from literature.
• Performs clinical chart reviews on a scheduled basis.
• Performs Patient Care Unit inspections on a scheduled basis.
• Prepares parenteral medications using sterile aseptic technique.
• Maintains adequate medication supply in the Pharmacy area and notifies supervisor of shortages.
• Intervenes and documents interventions to optimize pharmacotherapy.
• Reports adverse drug events and medication errors in the area served.
• Takes responsibility for the operation of the assigned Pharmacy area and its staff.
• Maintains pharmacological knowledge through reading and CE activities.
