Job summary

Beth Israel Lahey Health in Exeter, NH, is seeking a pharmacist to provide comprehensive pharmaceutical care, oversee dispensing and documentation, and support medication safety across the patient care teams.

Reporting to the Manager Pharmacy Operations, you will review orders, counsel on therapy, and ensure regulatory compliance while mentoring staff and contributing to quality initiatives.

Qualifications

• Pharm D. or BS Pharmacy with advanced training
• NH pharmacist license
• 1–3 years experience
• Pharmacy PG1 residency preferred

Responsibilities

• Review and monitor patient medication therapy for safety, efficacy, and appropriateness.
• Collaborate with physicians and healthcare providers on medication decisions and dosing.
• Provide clinical recommendations and document interventions.
• Prepare, label, verify, and dispense medications per physician orders and regulations.

• Maintain accurate dispensing records and oversee charging/crediting systems.

Reporting to the Manager Pharmacy Operations, provide pharmaceutical care services to the patients of Exeter Hospital including, but not limited to oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy as well as providing drug information, patient information and oversight of information and dispensing systems; ensure compliance with Federal, State and local laws and regulation as well as proper storage and documentation. Following physician's orders, reviews medication orders, compounds and dispenses medications and other pharmaceuticals.

• Standard, Rotating Shifts rotating shifts as needed & every 4th weekend

Key Responsibilities

• Review and monitor patient medication therapy and medication orders for safety, efficacy, and appropriateness based on patient-specific conditions and health status.
• Collaborate with physicians and other healthcare providers regarding medication selection, dosing, drug interactions, side effects, renal dosing adjustments, laboratory results, therapeutic drug monitoring, and adverse drug events.
• Provide clinical recommendations to the patient care team and document medication-related interventions.
• Prepare, label, verify, and dispense medications accurately and efficiently in accordance with physician orders and applicable regulations.
• Ensure compliance with federal, state, and local laws, accreditation standards, hospital policies, and controlled substance regulations.
• Maintain accurate medication dispensing records and oversee systems used to charge or credit medications and medication-related treatments.
• Ensure proper documentation and recordkeeping for all medications, with particular attention to controlled substances.
• Monitor and oversee medication-use processes, identify opportunities for improvement, and collect and present data, reports, and recommendations related to medication safety and operational effectiveness.
• Prepare and oversee sterile and non-sterile compounded medications in accordance with USP and USP guidelines.
• Supervise and collaborate with pharmacy technicians and support staff to ensure the accurate and timely delivery of medications to patient care areas.
• Participate in the orientation, training, and mentoring of pharmacy staff as appropriate.
• Contribute to quality improvement initiatives and support the delivery of safe, effective, and patient-centered pharmaceutical care.

Additional Skills & Qualifications

• Pharmacy PG1 residency preferred or BS Pharmacy with advanced training
• Doctor of Pharmacy (Pharm D.) or BS Pharmacy with advanced training
• 1-3 years experience
• Pharmacist – NH licensed

Pay Range

$51.15 - $83.89

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
