Job summary

VNA & Hospice of the Southwest Region in Bennington, Vermont, is seeking a per diem Personal Care Attendant (PCA) to assist in carrying out client care plans. The successful candidate will perform various patient care tasks, including daily living activities, and communicate effectively with patients and staff. Qualifications include a High School diploma, being at least 18 years old, and a valid driver's license. Experience in home care or hospice is preferred.

Qualifications

• Must be 18 years of age.
• Valid state driver's license, vehicle and automobile insurance required.
• Must show proof of auto insurance at time of employment.

Responsibilities

• Assist in carrying out a client plan of care.
• Collect and report patient and family data.
• Perform interventions and procedures to ensure safe quality of care.
• Perform various patient care tasks including daily living activities.
• Communicate clinical information effectively to patients and staff.
