Qualifications

• Accredited School of Medicine required.
• Completion of Fellowship Program in area of specialty.
• Licensed to practice as a physician in the state, and board certified (or board eligible within 5 years of training).
• Drug Enforcement Admin federal number.
• Cardiopulmonary Resuscitation (CPR) certification.

Responsibilities

• Practice of medicine within the scope of license, training and credentialing.
• Practice of medicine in accordance with current standards of care and clinic quality criteria.
• Assess, plan, implement and evaluate patient care across the life span.
• Performs other duties as assigned.
