Job summary

Mountainside Nursing Home in Mountainside, NJ is seeking an Occupational Therapy Assistant (Per Diem) to support our residents with personalized therapy plans. You will work closely with occupational therapists to promote independence, mobility, and well-being through targeted exercises and activities.

The role focuses on direct patient care, accurate documentation, and collaboration with the care team to ensure high-quality, consistent therapy services for our residents.

Qualifications

• Assisting with development and implementation of OT treatment plans.
• Providing direct patient care, including exercises and activities.
• Maintaining accurate records of patient progress and plans.
• Collaborating with interdisciplinary care team to ensure comprehensive care.

Responsibilities

• Provide occupational therapy services to residents in a post-acute care setting.
• Conduct initial and ongoing assessments to identify resident needs and goals.
• Develop and implement individualized treatment plans to address these needs and goals.
• Implement occupational therapy interventions, including exercises, activities, and modalities.
• Document resident progress, treatment plans, and other relevant information.
• Communicate effectively with the interdisciplinary care team, residents, and families.
