You will work under the supervision of a licensed therapist, contribute to the plan of care, and participate in the interdisciplinary team to deliver quality outcomes. The role emphasizes accurate documentation, ongoing communication of resident status, and adherence to state and federal guidelines.

Qualifications

• Graduate with an associate’s degree in physical therapy or occupational therapy.
• Active licensure/certification as a Physical Therapy Assistant or Certified Occupational Therapy Assistant.
• Prefer experience in older adult rehabilitation and knowledge of governmental/regulatory guidelines.
• Current state licensure in respective discipline.

Responsibilities

• Provide resident rehabilitation care under the plan of care.
• Collaborate with supervising therapist to communicate patient status.
• Complete required documentation per regulatory standards.
• Offer feedback to the rehabilitation team and director as needed.
• Supervise aides and participate in in-service training.
• Ensure compliance with state, federal, and accrediting guidelines.
