Job summary

Willow Creek Health Care Center in Clovis, CA is seeking a Per Diem Occupational Therapist to join our rehabilitation team. The OT will evaluate residents, develop customized treatment plans, and collaborate with physicians and staff to restore independence.

Qualifications

• Bachelor's degree in Occupational Therapy required; advanced degree preferred.
• Experience in a skilled nursing facility is preferred.
• Proficiency with documentation systems such as Net Health and PointClickCare.

Responsibilities

• Evaluate residents to determine OT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve ADLs.
• Supervise COTAs, aides, and students per state guidelines.
• Educate residents, families, and staff on therapy goals and home programs.
• Complete timely, accurate documentation per regulatory and payer requirements.
• Participate in care plan meetings and quality improvement initiatives.
