Job summary

Willow Creek Health Care Center in Clovis, CA seeks a dedicated Per Diem Occupational Therapist (OT) to join our rehabilitation team. You will evaluate residents, develop individualized treatment plans, and supervise COTAs and aides in a skilled nursing facility setting.

The ideal candidate holds a California OT license in good standing, with strong clinical, communication, and interpersonal skills. Per diem role with focus on quality outcomes, documentation, and team collaboration.

Qualifications

• Bachelor’s degree in Occupational Therapy required
• California OT license in good standing
• CPR certification if required and CE requirements must be met

Responsibilities

• Evaluate residents to determine OT needs and rehabilitation potential
• Develop and update individualized treatment plans to improve ADL independence
• Supervise COTAs, aides, and students per state guidelines
• Educate residents, families, and caregivers on therapy goals and home programs
• Maintain compliant documentation per regulatory and payer requirements
• Participate in care plan meetings and quality initiatives
