Job summary

Renewal Rehab is seeking a Per Diem Occupational Therapist in Peoria, Illinois. In this role, you will evaluate and treat patients with various impairments, create personalized care plans, and collaborate with a multidisciplinary team to ensure optimal outcomes.

The ideal candidate will have a degree in Occupational Therapy from an accredited program and be a licensed Occupational Therapist in Illinois. New graduates are encouraged to apply as mentorship opportunities are provided.

Qualifications

• Must be a licensed Occupational Therapist in the state of practice.
• Certified by the NBCOT as an OTR, if required by the state.
• New grads welcome. Mentorship available.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
