Job summary

Tender Touch Rehab Services in Hackensack, NJ is seeking an Occupational Therapist for per diem coverage in SNF and long-term care settings. Must hold an OT license and be NBCOT-certified as required by state, with flexible scheduling and competitive hourly pay.

Clinicians will evaluate and treat patients, develop individualized plans, and document care to meet regulatory requirements while collaborating with a supportive team and progressive professional development opportunities.

Qualifications

• Must have an OT degree from an AOTA-accredited program.
• NBCOT-OTR certification may be required by state.
• Current license as an Occupational Therapist in the state of practice.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure physician orders are obtained and up to date.
