Job summary

BrightStar Care is seeking flexible and compassionate Per Diem/On-Call Caregivers to provide one-on-one in-home care in Rochester, MN. This role offers schedule flexibility and potential for additional hours as client needs grow, while delivering dignity-centered, compassionate care in the comfort of clients' homes.

You will support daily living activities and collaborate with the care team to maintain independence and quality of life.

Qualifications

• Compassionate, patient and caring nature.
• Strong interpersonal and communication skills.
• Experience providing care in home health or similar settings.
• Reliable vehicle and valid driver's license with auto insurance.
• Negative TB test or cleared chest X-ray.
• Availability to work 2 weekend days per month.

Responsibilities

• Provide companionship and conversation to clients.
• Assist with daily activities (meals, light housekeeping, errands).
• Provide physical assistance (bathing, dressing, grooming, transferring, ambulation).
• Accompany clients to appointments and transport as needed.
• Monitor and report changes in health or needs.
• Coordinate with RN Case Manager to update the plan of care.

• Premium Short Shift Pay: We offer a variety of shift Lengths ranging from 1 hour to 12 hours. Our 1-hour shifts get paid at $30 per hour and hour 2-hour shifts get paid at $22.50 an hour. Any shift 3 hours or longer is paid at a base rate between $19-$22 based on experience.

Role Responsibilities:

• Provide companionship and conversation to enhance the lives of our clients.
• Assist with daily activities such as meal preparation, light housekeeping, and errands.
• Provide physical assistance such as bathing, dressing, grooming, transferring, and ambulation.
• Accompany clients to appointments or social outings, providing transportation as needed.
• Monitor and report changes in the client’s health, behavior, and needs.
• Act as a member of the Care coordination team to communication with the RN Case Manager to update the plan of care.

Requirements:

• A compassionate, patient, and caring nature.
• Strong interpersonal and communication skills.
• Experience providing care to clients in a hands-on environment (Home Health, Assisted Living, Group Home, Hospital Setting)
• Reliable vehicle and a valid driver's license with Auto Insurance.
• Negative TB test or Cleared Chest-Xray.
• Availability to work 2 weekend days per month.
