This is a great place to advance your nursing career as a CCM Nurse Practitioner / Physician Assistant, providing care to Optum members on a periodic or intermittent basis.

Primary Responsibilities :

  • Primary Care Delivery
  • Deliver cost-effective, quality care to assigned members
  • Manage medical and behavioral, chronic and acute conditions collaboratively with physicians or specialists
  • Perform assessments, document findings, and ensure documentation complies with CMS regulations
  • Ensure diagnoses are accurately ICD10 coded and documented
  • Address all quality elements and conduct medication reviews and reconciliations
  • Facilitate member care plans with facility staff, families, and providers
  • Evaluate and revise care plans based on effectiveness and member needs
  • Follow practice guidelines and protocols established by CCM
  • Complete mandatory training and travel between care sites as required
  • Care Coordination
  • Understand and communicate plan benefits and policies
  • Assess the necessity of ancillary services and coordinate member transitions
  • Monitor and adjust care plans as situations change
  • Review orders and interventions for appropriateness
  • Evaluate cost-effectiveness of care plans
  • Communicate effectively with all stakeholders
  • Promote the CCM program and develop membership
  • Exhibit creativity and independence in role execution
  • Participate in quality initiatives and maintain communication
  • Enter hours and demonstrate initiative towards goals

Qualifications - External :

  • Certified Nurse Practitioner or Physician Assistant with a national board certification
  • For NPs : MSN from an accredited program, board-certified through AANP or ANCC, with preferred certifications in adult-gerontology or family practice
  • For PAs : Graduated from an accredited PA program, NCCPA certified
  • Active, unrestricted license in your state
  • DEA licensure or ability to obtain post-hire
  • Reliable transportation and ability to handle physical demands
  • Availability for 24 hours / month, including off-hours
  • Ability to establish collaborative practice agreements if applicable

Preferred Qualifications :

  • 1+ years of post-grad experience in Long Term Care
  • Understanding of geriatrics and chronic illnesses
  • Experience with end-of-life discussions
  • Proficient in electronic medical records
  • Adaptability and flexibility
