Primary Responsibilities:
  • Primary Care Delivery
    • Deliver cost-effective, quality care to assigned members
    • Manage medical and behavioral, chronic and acute conditions, collaborating with physicians or specialists
    • Perform comprehensive assessments, document findings per CMS regulations
    • Ensure accurate ICD10 coding and documentation
    • Address all quality elements and conduct medication reviews
    • Facilitate care plan agreements with staff, families, and physicians
    • Evaluate and revise care plans as needed
    • Follow established practice guidelines
    • Complete mandatory training and travel between care sites
  • Care Coordination
    • Understand benefits, policies, and communicate effectively with stakeholders
    • Assess necessity of ancillary services and coordinate member transitions
    • Monitor and adjust care plans based on changing needs
    • Review treatment responses and optimize care plans
  • Program Enhancement & Behaviors
    • Maintain effective communication with all parties
    • Promote CCM program in facilities
    • Innovate and work independently with minimal supervision
    • Manage scheduling and participate in quality initiatives

Qualifications:
  • Certified Nurse Practitioner (MSN, board certified by AANP or ANCC), with preferred certifications in Adult-Gerontology or Family Practice
  • Active, unrestricted license in your state
  • Current or ability to obtain DEA license
  • Reliable transportation and physical ability to travel and lift 30 pounds
  • Collaborative practice agreement if required
Preferred Experience:
  • 1+ years in Long Term Care
  • Geriatrics, Chronic Illness, End-of-life discussions
  • Proficient in electronic medical records
  • Customer relationship skills and adaptability

The hourly rate ranges from $43.17 to $84.95, based on experience, location, and certifications. Benefits include comprehensive packages, incentives, stock options, and 401k.
