Job Description: CCM Nurse Practitioner / Physician Assistant (Per Diem)

$10,000 Sign-on Bonus for External Candidates

The hours are flexible - weekday availability preferred. Shifts can be 8-4, 9-5, or 10-6.

Serving millions of Medicare and Medicaid patients, Optum provides comprehensive health and wellness services. This role involves delivering periodic or intermittent medical care to Optum members.

Primary Responsibilities:
  1. Primary Care Delivery:
    • Provide cost-effective, quality care to assigned members.
    • Manage medical and behavioral, chronic and acute conditions collaboratively with physicians or specialists.
    • Conduct assessments, document findings per CMS regulations.
    • Ensure diagnoses are accurately ICD10 coded and documented.
    • Complete medication reviews and facilitate care plan implementation.
    • Evaluate and revise care plans as needed, utilizing established guidelines.
    • Attend mandatory training and complete all required educational modules.
    • Travel between care sites is mandatory.
  2. Care Coordination:
    • Understand and communicate benefits, policies, and procedures effectively.
    • Assess medical necessity of services and coordinate care transitions.
    • Monitor and adjust care plans based on patient needs and responses.
    • Review interventions for appropriateness and cost-effectiveness.
  3. Program Enhancement & Behaviors:
    • Communicate effectively with all stakeholders.
    • Promote the CCM program and develop membership.
    • Contribute creatively to improve methods and approaches.
    • Operate independently with minimal supervision.
    • Enter available hours into scheduling systems at least one month prior.
    • Participate in quality initiatives and demonstrate proactive behavior.
    • Check emails intermittently for updates and training.

Required Qualifications:
  • Certified Nurse Practitioner through a national board, with a master's degree in Nursing (MSN).
  • Active, unrestricted license in your state.
  • Current DEA licensure or ability to obtain it.
  • Reliable transportation for site visits.
  • Ability to lift 30 pounds, navigate stairs, and adapt to various dwelling conditions.
  • Ability to gain a collaborative practice agreement if required by your state.
Preferred Qualifications:
  • At least 1 year of post-graduate experience in Long Term Care.
  • Knowledge of geriatrics, chronic illness, and end-of-life discussions.
  • Proficient in electronic medical records and developing positive patient relationships.
  • Adaptable to changing environments.

The hourly pay ranges from $43.17 to $84.95, based on experience, education, certifications, and location. Benefits include a comprehensive package, incentives, stock purchase options, and 401k contributions.
