Job summary

Optum is seeking a Per Diem Nurse Practitioner or Physician Assistant to support patients in skilled nursing and transitional care within the Patient Connect Program. You will perform one-time comprehensive health assessments in collaboration with care teams and case managers.

The schedule is per diem with a minimum of 10 hours per week, offering flexible days, evenings, and weekends. Field-based, autonomous practice with an emphasis on care transitions and reducing hospital readmissions.

Qualifications

• Active NP or PA license.
• National certification (AANP, ANCC, NCCPA, etc.).
• DEA license (or ability to obtain).
• Valid driver’s license and reliable transportation.
• Ability to travel to skilled nursing facilities within the assigned territory.
• Ability to work at least 10 hours per week.

Responsibilities

• Conduct comprehensive patient assessments.
• Collaborate with physicians, nursing staff, families, and case managers.
• Support care coordination and transition planning.
• Complete documentation, coding, and quality gap closure activities.
