Qualifications

• Specialized Diploma graduate from an accredited school of nursing.

Responsibilities

• Collaborates with patients and families to develop individualized care plans and address priorities.
• Educates patients and families to identify learning needs and preferences for understanding care and treatment.
• Uses information technology to communicate, manage knowledge, mitigate errors, and support ethical decision making.
• Works within nursing and inter-professional teams to provide safe, compassionate care and respect patient preferences.
• Participates in safety and quality improvement efforts and supports ongoing professional growth.
• Contributes to discharge planning and post-discharge follow-up arrangements.
