Job summary

BAYADA Home Health Care is seeking a Licensed Medical Social Worker (MSW) for a per diem opportunity to perform home health visits across Dauphin and Cumberland counties. The role supports a multidisciplinary team to provide skilled nursing and rehabilitative care at home.

Responsibilities include home visits, social evaluations, community resource referrals, and patient advocacy. Basic EMR use and travel to cases are required. Per diem schedule offers flexible hours and competitive pay.

Qualifications

• Must be licensed by the Pennsylvania Board of Social Work Examiners at the LMSW or LISW level.
• Minimum of a Master of Social Work with at least 2 years of community-based experience.
• CSWE accreditation demonstrated by transcript or degree.
• Home care experience is a plus.
• Strong reading, writing and verbal communication in English.
• Ability to work independently and manage time effectively.
• Solid interpersonal and computer skills; travel to cases as assigned.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Assist the team in understanding social and emotional factors related to clients' health problems.
• Provide clinical social evaluations and plan interventions based on evaluation findings.
• Identify community resource referrals for client and family needs.
• Advocate for the client and family to navigate the patient through the community system.
• Participate in the development of the total plan of care with the multidisciplinary team.
• Develop and maintain contracts with public and private agencies as resources for the patient.
• Educate patients and families about community resources to promote health and independence.
