Job summary

BAYADA Home Health Care is seeking an energetic Licensed Medical Social Worker (MSW) for a per diem role performing home health visits in Vermont. This position serves clients across designated Franklin, Orange and Washington Counties with a multidisciplinary care approach.

The ideal candidate will hold an MSW, be able to travel, and communicate effectively in English. Prior home care is a plus; EMR experience is preferred, and PPE protocols are in place for safety.

Qualifications

• Masters in Social Work required; 2+ years of community-based experience.
• CSWE accreditation demonstrated by transcript or degree.
• Home care experience a plus; strong communication skills.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Assess social and emotional factors related to health problems.
• Provide clinical social evaluations and plan interventions.
• Identify community resource referrals for patient needs.
• Advocate for client and family to navigate the system.
• Participate in care plan development with the multidisciplinary team.
• Develop and maintain contracts with public and private agencies.
• Educate patients and families about community resources.
• Document observations and interventions using EMR.
