Hartford HealthCare’s Neighborhood Health program seeks passionate per diem Advanced Practitioners to join our primary care team focused on health equity in underserved communities. You will deliver high-quality preventive and urgent care via mobile and community-based services across Connecticut.

Role is open to Family Nurse Practitioners and Physician Assistants. Travel to home base hubs in Hartford/New Britain, then to clinic sites and Street Medicine; 100% outpatient Family Medicine;

Qualifications

• Licensure: Unrestricted State of Connecticut Medical License (or eligible).
• Training: Family Nurse Practitioner (FNP) or Physician Assistant (PA).
• Experience: 2+ years as a Family Nurse Practitioner or Physician Assistant, preferably in a primary care or urgent care setting.

Responsibilities

• Perform routine physical examinations and develop medical, educational, and psychosocial care plans.
• Collaborate with physicians and care team for further evaluation or referral as needed.
• Provide appropriate treatment within the provider’s scope of practice and escalate concerns to physicians.

Skills

Unrestricted CT medical license

FNP or PA training

2+ years experience as FNP/PA

Valid driver’s license within 3 months

Ability to push/pull/lift 50 lbs
