Job description

Responsibilities
  • Provides primary care services to patients on CHSI mobile health units and outreach sites.
  • May assist the Medical Director in the revision and implementation of practice principles, clinical protocols, and health care plans as needed.
  • Consults with staff nurse practitioners and physician assistants as directed.
  • Follows all applicable national patient safety goals in the provision of patient care.
  • Serves as a role model for effective teamwork among staff and actively works to maintain positive morale and attitude; leads the team in the team-based care model.
  • Facilitates coordination and follow-up care between mobile health services and in-clinic providers to support continuity of patient care as needed.
  • Provides care in collaboration with the center's team of physicians, nurse practitioners, behavioral health providers, nurses, and medical assistants.
  • Serves as a resource person for clinical staff regarding patient care issues.
  • Establishes and maintains rapport with community agencies.
  • Demonstrates sincere dedication and loyalty to the philosophy and mission of CHSI.
Physical Abilities
  • In an eight‑hour workday, must be able to bend and crouch occasionally, sit continuously, and stand or walk frequently.
  • Must be able to lift up to 50 pounds occasionally.
  • Must be able to use hands/arms, feet/legs for repetitive fine to large motor skill movements.
Qualifications
  • Graduate of an accredited Doctor of Medicine or Doctor of Osteopathic Medicine program.
  • Minimum of 2 years of experience in a community health or primary care setting.
Knowledge, Skills, and Abilities
  • Ability to provide effective care consistent with medical best practices.
  • Ability to see all ages of patients, including pediatrics and geriatrics.
  • Ability to provide prenatal care in an outpatient setting.
  • Ability to perform women's health procedures including LARC placement, endometrial biopsy, and possibly colposcopy.
  • Ability to provide healthcare in an unbiased manner to patients of many cultural backgrounds.
  • Ability to work independently.
Licensure and Credentials
  • Licensure and certification as required by the discipline.
  • MN or ND state license.
  • DEA and controlled substance license.
  • BLS certification.
