Qualifications

• Minimum of 2 years of experience in community health or primary care setting.
• Licensure and certification as required by the discipline.
• MN OR ND state license, DEA and controlled substance license, BLS Certification.

Responsibilities

• Provides primary care services to patients on CHSI mobile health units.
• Facilitates coordination and follow-up care between mobile health services and in-clinic providers.
• Consults with staff nurse practitioners and physician assistants as directed.
