Job summary

Masonicare at Wesley Heights in Shelton, CT is seeking an LPN for per diem shifts to provide nursing care under supervision, and to support resident services plans and collaborative care delivery.

Qualifications

• Graduate of accredited School of Nursing.
• 1–2 years clinical experience in hospital or similar setting preferred.
• Current LPN license; CPR certification.

Responsibilities

• Contributes to resident care plans by data collection and care planning.
• Communicates with residents, families and health care team to meet needs.
• Educates and counsels residents and families on AL procedures and care needs.
• Documents information in the client service record in a timely manner.
• Conducts supervisory visits of care staff as required.
• Participates in educational programs and meetings to enhance skills.
• Oversees medication management in collaboration with physicians and pharmacists.
• Performs other duties as assigned.
