Job summary

Masonicare at Greenridge Place in Rocky Hill, CT is seeking a Per Diem LPN to provide nursing care for a select group of residents under supervision. The role emphasizes collaboration with SALSA/RN designee, physicians, pharmacists and outside agencies to support comprehensive resident services.

The candidate will participate in care planning, document accurately, and supervise care staff while maintaining current licensure and CPR certification.

Qualifications

• Must be a graduate of an accredited School of Nursing.
• 1-2 years of clinical experience in a hospital or similar health care setting preferred.
• Current LPN license; CPR certification.

Responsibilities

• Provides nursing care and supports resident services plan.
• Communicates with residents, families and health-care team to meet needs.
• Educates, supervises and counsels residents and families on care needs.
• Documents information accurately in the client service record in a timely manner.
• Conducts supervisory visits of care assistants and aides as required.
• Participates in ongoing education and professional development activities.
• Oversees medication management with supervision, storage, reconciliation and preparation for administration.
• Performs other duties as assigned.

Under the supervision of a SALSA, the LPN provides nursing care for a select group of residents by providing supportive and restorative care, teaching and collaborating in the implementation of the resident services plan.

LPN - Essential Duties And Responsibilities

• Contributes to the resident care and service plans by collecting data based on age, needs and abilities in an ongoing and systematic manner and participating in regular care planning meetings.
• Communicates and collaborates effectively with residents, families, colleagues and other members of the health care team to meet resident needs.
• Provides education, supervision and counseling to resident and family regarding AL procedures and other care needs.
• Documents accurate, pertinent and comprehensive information in a timely and concise manner in the client service record.
• Conducts supervisory visits of resident care assistants, companions and homemakers as required.
• Participates in educational programs, inservices, meetings and workshops and utilizes current literature to enhance professional skills and knowledge.
• Under supervision of SALSA/RN DESIGNEE and in collaboration with physicians, pharmacists and outside agencies, will be responsible for oversight of medication management including but not limited to ordering, storing, reconciling and preparing medications for administration and/or CNA prompt.
• Performs other duties as assigned.

Minimum Qualifications

Education: Graduate of accredited School of Nursing.
Experience: 1-2 years-clinical experience in a hospital or similar health care setting preferred.
Certificates, Licenses, Registrations: Current LPN license; CPR certification Registration.
