Job summary

Legacy Hospice is seeking Per Diem Compassionate Licensed Practical Nurses (LPN) to deliver direct patient care in and around the Norfolk area. The LPN will follow the plan of care under RN supervision, provide treatments, and collaborate with the care team to achieve positive patient outcomes.

The role involves per diem scheduling in Newport News and Norfolk, with responsibilities including death support where required and ongoing communication with the care team.

Qualifications

• Graduate of an Accredited Practical Nurse or Vocational Nurse Program.
• Current unencumbered LPN license in the state of practice.
• Minimum three years' LPN experience in community health or medical-surgical/acute care settings.
• Hospice/home health experience preferred.
• CPR certification if required by state.

Responsibilities

• Assess and reevaluate patient status per the plan of care with RN Case Manager.
• Provide focused physical assessments considering physical, emotional, and spiritual factors.
• Attend and support at time of death as required by physician's order and local regulations.
• Maintain regular communication with the attending physician and RN Case Manager; document changes.
• Document clinical records timely and accurately in the electronic record.
