Legacy Hospice in the Norfolk area is hiring Per Diem Compassionate Licensed Practical Nurses to provide direct patient care under RN supervision, following the plan of care and collaborating with the care team to achieve positive patient outcomes. The role requires an LPN license, a minimum of three years’ experience in community health or acute care, and EMR proficiency.

Hospice/home health experience is preferred but not required; CPR may be required by state.

Qualifications

• Graduate of an Accredited Practical Nurse or Vocational Nurse program.
• Holds current unencumbered LPN license in state of practice.
• Minimum three years’ LPN experience in community health or acute care.
• Hospice/home health experience preferred but not required.
• Proficient in typing and EMR usage.
• CPR certification as required by state.

Responsabilités

• Assesses and reevaluates patient status per plan of care with RN involvement.
• Provides focused physical assessments addressing physical, emotional and spiritual needs.
• Attends and supports at the time of death per physician orders.
• Maintains timely, accurate clinical documentation in the EMR.
