Qualifications

• Valid LPN license in New Hampshire or a multistate compact license with NH authority.
• Strong clinical skills and a compassionate approach to patient care.
• Excellent communication and interpersonal skills.
• Ability to work effectively both independently and as part of a team.
• Experience in long-term care or similar setting preferred but not required.

Responsibilities

• Provide direct nursing care to residents per established policies and standards.
• Administer medications and treatments as ordered.
• Monitor residents and promptly identify and report changes in condition.
• Complete accurate and timely nursing documentation.
• Communicate with physicians, residents, families, and the care team.
• Assist with development and implementation of individualized care plans.
• Supervise and guide nursing assistants and other staff as appropriate.
• Ensure resident rights, privacy, safety, and independence are maintained.
• Follow infection prevention, safety, and regulatory requirements.
• Respond appropriately to resident needs and changes in condition.
• Work collaboratively with the nursing team to provide exceptional resident care.
