TRU Healthcare is seeking a Per Diem Licensed Practical Nurse (LPN) to provide home health services in Barnum, MN. The role offers per-visit compensation with the flexibility to work around your schedule and travel between patient visits.

Qualifications

• CPR/BLS certification required.
• Home health experience preferred.
• Strong clinical, communication, and organizational skills.

Responsibilities

• Provide skilled nursing services to patients in their homes according to established care plans and physician orders.
• Monitor patient conditions, identify changes in health status, and communicate concerns to the care team.
• Administer treatments as ordered and within the scope of practice.
• Perform nursing assessments and provide appropriate interventions based on patient needs.
• Educate patients and caregivers on medications, treatments, disease management, safety, and other aspects of their care.
• Accurately and promptly document patient care and communicate with the interdisciplinary team.
• Build meaningful relationships with patients and families while providing compassionate, patient-centered care.
