Atlas Healthcare, LLC is seeking a compassionate Per Diem Licensed Practical Nurse (LPN) to join Shrewsbury Rehabilitation and Nursing at Southgate in Shrewsbury, Massachusetts. You will provide direct nursing care, supervise CNAs, and collaborate with the care team to ensure excellent resident outcomes.

The role requires a current Massachusetts LPN license, experience in long-term care, strong communication, and the ability to make sound clinical decisions in a fast-paced environment.

Qualifications

Responsibilities

• Provide direct nursing care, treatments, and medication administration.
• Supervise and support CNAs during daily care activities.
• Monitor resident conditions and report changes.
• Assist with admissions, transfers, and discharges.
• Complete accurate and timely nursing documentation and incident reports.
• Maintain infection control, safety, and regulatory compliance standards.
• Support a respectful, team-oriented environment for residents, families, and staff.
• Follow facility policies and individualized care plans.
