Job summary

VNS Health is seeking a Per Diem Social Worker to join our home care team in New York. The role involves performing psychosocial assessments, developing care plans, and delivering direct services in patients' homes and facilities.

Responsibilities include counseling, crisis intervention, advocacy, and collaboration with the care team to ensure optimal patient outcomes. A NYS social work license and MSW are required, with bilingual Chinese (Mandarin or Cantonese) strongly preferred.

Qualifications

• MSW degree required or equivalent, and NYS licensure as Social Worker.
• Minimum of 1 year social work experience in health care setting.
• Bilingual Chinese (Mandarin or Cantonese) required.

Responsibilities

• Assess psychosocial status and needs of clients and families.
• Develop and revise the plan of care with mutually agreed goals.
• Travel to patients' homes or care facilities to deliver direct services.
• Provide short-term counseling, resource planning, and crisis intervention.
• Advocate for patient/family entitlements and community services.
• Document clinical notes and ensure accurate charting per policy.
• Participate in discharge planning and care coordination with the team.
