Job summary

Select Specialty Hospital, a Critical Illness Recovery LTACH, is seeking a PRN Case Manager. Licensed Social Workers Only. Pay: $52/hr.

Availability to work a minimum of 2 shifts per month on an as needed basis - shifts scheduled on weekdays, 8a.m.–5p.m. You will perform discharge planning, utilization reviews, treatment plan management and financial management, reporting to the Director of Case Management and providing social work services per state guidelines.

Qualifications

• Current licensure as a social worker is required, with experience in discharge planning.
• Minimum experience in an acute medical case management setting.

Responsibilities

• Develops and implements a patient-specific, safe discharge plan.
• Performs utilization criteria reviews and coordinates with payors.
• Coordinates UR compliance with Quality Management and maintains fiscal responsibilities.
• Facilitates multidisciplinary team meetings with physicians and therapists.
