Aveanna Healthcare is seeking a Licensed Practical / Vocational Nurse (LPN/LVN) in Chicago, IL to provide private duty home health care, delivering compassionate, patient-centered services in patients' homes.

Qualifications

• Must have and maintain an active, unencumbered LPN/LVN license in the state of practice.

• Reliance on reliable transportation

Responsibilities

• Provide compassionate homecare nursing to patients in their homes.
• Collaborate with clinicians to achieve outstanding clinical outcomes.
• Utilize tablet in patient homes for electronic documentation.
• Support career development and paid training opportunities.

  • Compact licenses must be transferred to your state of residence within 60 days
• Current CPR certification (with hands-on component)- Aveanna can assist in obtaining this requirement after hire, if necessary.

• Must have reliable transportation
