University of California, San Francisco is seeking an Inpatient/Acute Care Per Diem Pharmacist to provide comprehensive pharmaceutical care across UCSF Health System locations. Requires a California pharmacy license in good standing and a current AHA Basic Life Support (BLS) certification.

Qualifications

Must have graduated from an accredited school of pharmacy and possess a Doctor of Pharmacy degree (PharmD) or equivalent degree as determined by the National Board of Pharmacy.
For graduates in a non-US country or jurisdiction, pharmacists must have earned a Foreign Pharmacy Graduate Examination Committee (FPGEC) Certification as determined by the National Board of Pharmacy.

Responsibilities

Provide pharmaceutical care to patients at all acute care pharmacy locations within the UCSF Health System.
Obtain medication histories and review/assess medication profiles.
Provide drug information to patients and health care providers; collaborate with clinicians on drugs of choice and therapeutic plans.
Educate patients on proper use of medications and ensure understanding of cautions, adverse reactions and interactions.
Assist in oversight of pharmacy technicians and students as needed.
