Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in the Cheyenne, WY area and surrounding communities. The role focuses on direct patient care, education, and coordination with the care team in home or alternate sites of care.

You will assess patients, develop care plans, participate in on-call rotations, and ensure documentation meets policy and state guidelines. Travel is required with mileage reimbursement.

Qualifications

• Current RN license by the state's Board of Nursing, in good standing
• RN degree, bachelor’s degree in nursing (BSN) from an accredited institution preferred
• Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required
• Participation in an on-call rotation, including nights and weekends
• Administrative Oversight: monitor workflows, ensure policy compliance, manage resources
• Emergency Availability: respond promptly to emergencies, including nights, weekends, holidays

Responsibilities

• Provide direct care for assigned patients including initiation and maintenance of communications with care team members
• Evaluate and educate patient/caregiver on home infusion therapy safety
• Assess home environment and implement safety interventions for prescribed care
• Develop and evaluate the plan of care with the team and monitor therapy response
• Manage scheduling via company platform and communicate changes
• Coordinate patient care via approved company platform
• Participate in the nurse on-call rotation to support customers
• Document all patient encounters and care activities per policy and regulatory requirements
• Maintain knowledge of Soleo therapies, protocols, and clinical standards
• Ensure timely submission of POTs and required clinical documents per policy
