Job summary

Saint Francis Hospital and Medical Center in Hartford, CT is seeking a part-time, per diem LCSW/LMSW to join the Case Management team. The social worker will assess patients, coordinate discharge planning, and provide direct social work intervention throughout hospitalization.

The role requires an MSW and state licensure consideration (LMSW/LCSW). CCM/ACCC certifications are preferred, with knowledge of Medicare/Medicaid guidelines and care coordination experience valued.

Qualifications

• MSW degree from an accredited School of Social Work.
• CT State Social Work Licensure LMSW or LCSW preferred.
• CCM certification and ACCC preferred.
• Knowledge of Medicare, Medicaid, and managed care regulations.
• Experience with utilization management, home care, community resources, and rehabilitation beneficial.

Responsibilities

• Provide ongoing case management support to the patients.
• Assesses patient's needs and develops discharge care plans.
• Communicate effectively with staff, colleagues, patients, and family members.
• Utilizes standard Social Work intervention techniques collaborating with patients and families.
• Work as a resource to other staff members.
