Trinity Health Corp. is seeking a per diem LCSW/LMSW to join the Saint Francis Hospital and Medical Center Case Management team in a part-time, on-site role.

The Social Worker will assess patients, provide direct social work services, and help with discharge planning throughout admission and discharge. The ideal candidate holds an MSW from an accredited school, CT LMSW/LCSW licensure is preferred, and CCM/ACCC certifications are advantageous.

Qualifications

• MSW from an accredited School of Social Work.
• CT State Social Work Licensure LMSW or LCSW preferred.
• CCM and ACCC certifications preferred.

Responsabilités

• Provide ongoing case management support to the patients.
• Assesses patient's needs and develops discharge care plans.
• Communicate effectively with staff, colleagues, patients, and family members.
• Utilizes standard Social Work intervention techniques collaborating with patients and families.
• Work as a resource to other staff members.
