Stoughton Health is seeking a Pharmacist to join our Pharmacy team on a per diem basis at our Stoughton Hospital location. The Pharmacist will prepare, dispense, and manage medications for inpatient and outpatient care, ensuring safe and effective use of drugs across age groups.

The role involves collaborating with physicians, nurses, and colleagues to optimize patient outcomes, while adhering to policies and regulations.

Competenze

• Required: Bachelor's Degree in Pharmacy.
• Preferred: Doctorate in Pharmacy.
• Licensure: Registered as a Pharmacist with the Wisconsin Department of Safety and Professional Services.
• Current BLS certification at hire and ACLS within 6 months of hire.
• Computer skills: basic computer skills and access to electronic health records.

Mansioni

• Review, prepare, and dispense medications accurately.
• Monitor medication therapy to ensure appropriate regimens.
• Complete admission and discharge medication reconciliation.
• Collaborate with medical providers on drug choices, dosages, interactions, and side effects.
• Ensure compliance with hospital policies, laws, and regulatory standards.
• Provide direct supervision of pharmacy technicians, interns, and students.
