Job summary

Atlantic Health System is seeking a Per Diem Social Worker for Case Management at Chilton Medical Center in Pompton Plains, NJ. The role focuses on discharge planning, collaboration with the care team, and coordinating resources for patients and families.

Responsibilities include facilitating discharges, obtaining authorizations, and ensuring accurate documentation. The ideal candidate will have MSW and NJ LSW credentials and at least 1 year of medical setting experience.

Qualifications

• MSW and NJ LSW required.
• 1-year medical setting (acute care hospital) or 1-year externship preferred.

Responsibilities

• Provide Social Work coverage to facilitate discharges, meet with patients and families to assess and provide discharge planning support, resources, referrals, transfers to Extended Care and inpatient psychiatric facilities.
• Work closely with the Healthcare Team to implement the plan of care, obtain insurance authorizations as required, communicate with community resources and area facilities.
• Phone, Fax and Electronic documentation a must.
