Job summary

Crescent Hospice is seeking a dedicated Licensed Master Social Worker to join our per diem team serving the Aiken, SC area. You will deliver compassionate social work services to hospice patients and families, offering counseling and support to cope with end-of-life challenges.

Responsibilities include psychosocial assessments, coordination with the RN Case Manager and interdisciplinary team, and advocacy to ensure personalized, dignified care.

Qualifications

• Master of Social Work from an accredited school
• Licensed as a Social Worker by the State Board
• Current Driver's License and reliable vehicle

Responsibilities

• Assess psychosocial needs and support patients and families
• Coordinate with RN Case Manager and interdisciplinary team
• Provide counseling and advocacy to support emotional well-being
• Evaluate home situations and resources to support care
• Participate in care planning and family meetings
