Job summary

Optum is looking for a Social Worker - Hospice Per Diem to provide medical social services and assist patients and families in understanding significant social and emotional factors affecting health.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work.
• 1+ years of social work experience in a health care setting.
• Current Driver’s License and access to a dependable vehicle.

Responsibilities

• Assist patients and families in understanding social factors related to health status.
• Contribute to the development of a comprehensive Plan of Care.
• Instruct team members on community resources available to assist patients.
• Function as Bereavement Coordinator if needed.
