Providence at Home with Compassus is seeking a Registered Nurse to plan, organize and direct hospice care in patients' homes within Napa, CA. The role emphasizes community health education and collaboration with the IDG to tailor care plans for individuals and families.
The RN will perform comprehensive assessments, develop Plans of Care, administer prescribed treatments, and provide guidance to patients and caregivers.

The Registered Nurse plans, organizes and directs Hospice care and is experienced in nursing, with an emphasis on community health education / experience. The Hospice nurse builds from the resources of the Interdisciplinary Group (IDG) to plan and direct services to meet the needs of individuals and families within their home environments. May provide patient care young, middle and older adults.

California (CA) Required: Education sufficient to meet certification, licensure, or registration requirements.
Preferred: 1 year of recent experience in hospice, palliative care, home health, acute care, and/or skilled nursing as a registered nurse.
Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications. Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
Other Skills and Abilities: Excellent observation, verbal and written communication, problem solving and basic math skills. Visual acuity and hearing to perform required nursing skills. Preferred working knowledge with computer applications, i.e. Microsoft Word/e-mail.
Required within 30 days of hire: National Provider BLS – American Heart Association (Vendor Managed) Active state RN license (Vendor Managed). Valid driver’s license State Specific Requirements

The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. This position involves providing care in patient homes and in contracted facilities such as skilled nursing facilities, residential care facilities for the elderly, assisted living, memory care, or board and care facilities. This position will require driving potentially 30 or more miles in a day.

Compensation range: $60.47 - $80.07 The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
