Job summary

AdvisaCare Hospice is seeking a compassionate Hospice RN to join our Gilbert, AZ team serving the West Valley and surrounding areas. You will provide emotional support for patients and families, conduct supervisory visits, and participate in interdisciplinary group meetings to develop and review the patient/family plan of care.

We welcome bilingual and Spanish-speaking candidates. Current Arizona RN license in good standing is required, with full-time on-call availability and a patient-centered,

Qualifications

• Active Arizona RN license in good standing.
• Hospice experience preferred.
• Empathetic approach to patients and families.
• Ability to work full-time on-call.

Responsibilities

• Initial and ongoing assessment of patient’s needs.
• Implement and revise the plan of care.
• Ensure compliance with hospice policies and procedures.
• Educate patient/family on disease process and end-of-life care.
• Supervise paraprofessionals providing services to patient.
• Prepare clinical notes and participate in IDG conferences.
• Support performance improvement initiatives and maintain high client satisfaction.
