AdvisaCare is searching for a compassionate and dynamic Per Diem Hospice RN to join our dedicated team in Mount Pleasant, Michigan. In this role, you will provide emotional support to patients and their families, ensuring dignity and comfort in their final days. Responsibilities include conducting assessments, implementing care plans, and supervising caregivers. A current Michigan nursing license and a minimum of one year of hospice experience are required. Responsibilities

• Conduct initial and ongoing assessments of patients.

• Supervise and consult with paraprofessionals providing patient services.

Responsibilities

As a Per Diem Hospice RN, you will provide emotional support and ensure patients live their final days with dignity and comfort. Your responsibilities include:

• Initial and on-going assessment of patient’s physical, functional, psychosocial and environmental needs
• Implement the individualized plan of care and revisions as needed
• Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance
• Consult and educate the patient/family on disease process, self‑care techniques, end‑of‑life care, ethical issues
• Supervise paraprofessionals providing services to patient
• Prepare clinical and progress notes, participate in IDG conferences
• Participate in Hospice performance improvement program
• Maintain high level of customer satisfaction with our clients, families and caregivers
• Communicate information using current process and technology within the organization

Requirements

• Current Michigan License in good standing
• Minimum 1 year hospice experience
• Empathetic approach to patients and caregivers
• Participate in field visits daily, weekly office meetings as indicated
• Ability to adapt to patient’s status and needs on a full time on call basis
