Job summary

Pathways Healthcare in Kingston, PA is seeking a licensed registered nurse to join our in‑home health and hospice team. You will perform preventive and rehabilitative nursing procedures, assess patients and families, administer medications, and develop plans of care in partnership with physicians.

You will collaborate with interdisciplinary teams, supervise home health aides, and support discharge planning.

Qualifications

• Active nursing license in the state where the position is based.
• Preferably 2+ years of nursing experience with geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable, insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create Plans of Care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them as part of the care plan.

Benefits & Compensation

• Pay Structure: Paid per visit ($50 weekdays / $55 weekends & holidays). Training, meetings, and other hourly activities are paid at $35/hr.
