Job summary

Qualifications

• 1 year+ experience in post-acute, acute care and home health.
• In-depth nursing operations knowledge and med administration.
• Familiar with safety regulations and patient safety practices.
• Strong observation skills and attention to detail.
• High ethical standards and patient-centered approach.
• Excellent communication and compassion in care.
• Formal nurse training and license required.
• Current CPR certification.

Responsibilities

• Provide high-quality, patient-centered care in the home setting.
• Build meaningful relationships with patients and families.
• Deliver nursing services per agency policy and plan of care.
• Observe patients and report adverse reactions to meds or treatment.
• Assist physicians and RNs in carrying out the plan of care.
• Instruct patients and families on diet, medication, and treatment.
