Job summary

Mission Hospice & Home Care in San Mateo, CA seeks a per diem Hospice Nurse Case Manager to assess and plan nursing care for patients and families. The role covers San Mateo County with occasional coverage in Santa Clara County, following CDPH vaccine requirements for healthcare personnel.

The position requires an active California RN license, 1 year of recent nursing experience or a BSN, and preferred Home Health/Hospice exposure.

Qualifications

• One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing.
• Experience in home health and/or hospice setting preferred.

Responsibilities

• Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions
• Coordinate all patient/family services and prioritization of needs with the transdisciplinary team
• Use a case management approach and refer to other services as needed
• Coordinate the implementation of the plan of care and ensure care coordination and case management for each patient
• As applicable, apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and patient’s eligibility

The Hospice Nurse Case Manager is responsible for the assigned caseload or facility where they will be providing skilled nursing assessment, planning and care to maximize the comfort and health of patients and families.

Schedule

Sunday, Thursday & Friday; 8:30am – 5:00pm

Service Area

San Mateo County with additional coverage to Santa Clara County as needed

Essential Duties & Responsibilities include (but are not limited to):

• Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions

• Use a case management approach and refer to other services as needed
• Coordinate the implementation of the plan of care and ensure care coordination and case management for each patient
• As applicable, apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and patient’s eligibility

Education/Experience

• One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing
• Experience in home health and/or hospice setting preferred

Certificates, Licenses, Registrations

• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable
• HPNA certification preferred

Pay Range

$58.00 – $73.65 hourly
