Job summary

Mission Hospice & Home Care is seeking a Hospice Nurse Case Manager for a per diem role in San Mateo, CA. The RN will assess, plan and deliver skilled nursing care to maximize comfort and health for patients and families.

Schedule involves Sunday, Thursday and Friday, 8:30am–5:00pm, covering San Mateo County with potential Santa Clara coverage. California RN licensed candidates with hospice/home health experience are preferred.

Qualifications

• One year experience as a professional nurse in the past three years.
• Experience in home health and/or hospice setting preferred.
• Current license as a registered professional nurse in the State of California.

Responsibilities

• Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions
• Coordinate all patient/family services and prioritization of needs with the members of the transdisciplinary team
• Use a case management approach and refer to other services as needed
• Coordinate the implementation of the plan of care and ensure care coordination and case management for each patient
• As applicable, apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and patient’s eligibility

The Hospice Nurse Case Manager is responsible for the assigned caseload or facility where they will be providing skilled nursing assessment, planning and care to maximize the comfort and health of patients and families.

San Mateo County with additional coverage to Santa Clara County as needed

Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

• Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions

Education/Experience

• One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing
• Experience in home health and/or hospice setting preferred

Certificates, Licenses, Registrations

• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable
• HPNA certification preferred

Pay Range

$58.00 – $73.65 hourly
