Job summary

Providence Hospice and Palliative Care in St. John, IN seeks a Nurse Practitioner per-diem to provide primary hospice care. Independent practice with physicians, conducting visits, and documenting in CMS-required formats. Collaboration with MSW, RN, and IDG to coordinate comprehensive end-of-life care.

Includes on-call evenings, weekend availability, and mileage reimbursement. On-site visits across Lake and Porter counties with variable pay by visit, plus training hour pay.

Qualifications

• Must hold a valid Indiana NP license.
• Experience in hospice or home health preferred.

Responsibilities

• Provide primary care for hospice patients independently and with physicians.
• Conduct initial and follow-up visits, and complete required CMS face-to-face documentation.
• Coordinate care with multidisciplinary team to meet end-of-life goals.
• Order medications, lab tests, and diagnostic studies as appropriate.
• Communicate effectively with patients, families, and IDG team to align care plans.
