Job summary

Legacy Hospice in Winchester is seeking Per Diem Licensed Practical Nurses to deliver direct patient care under RN supervision. You will follow the care plan, provide treatments, and work with the team to achieve positive patient outcomes.

The role requires LPN licensure, 3+ years in relevant settings, EMR proficiency, CPR as applicable, and a compassionate, patient-centered approach. Per Diem schedule in the Winchester area.

Qualifications

• Graduate of an Accredited Practical Nurse or Vocational Nurse Program.
• Holds current unencumbered LPN license in state of practice.
• Minimum three years' LPN experience in community health or medical-surgical/acute care.
• Hospice/home health experience preferred but not required.
• Computer proficiency including typing and EMR background.
• Current CPR certification if required by state.

Responsibilities

• Assesses and evaluates patient's status by following the plan of care with RN Case Manager.
• Provides focused physical assessments considering physical, emotional and spiritual aspects.
• Attends and supports at the time of death with physician's order and local regulations.
• Maintains communication with attending physician and RN Case Manager and reports changes.
• Monitors and documents clinical records to ensure timeliness, accuracy, and completeness in the EMR.
