Job summary

Legacy Hospice in Winchester is seeking Per Diem Compassionate Licensed Practical Nurses to deliver direct patient care under RN supervision. You will follow plans of care, participate in treatment, and support a patient-centered end-of-life experience.

The role emphasizes collaboration with the care team, accurate documentation, and flexible per diem scheduling in the Winchester territory.

Qualifications

• Graduate of an Accredited Practical Nurse or Vocational Nurse Program.
• Holds current unencumbered LPN license in the state of practice.
• Minimum three years' LPN experience; hospice/home health preferred.
• Computer proficiency with EMR systems; CPR certification may be required.

Responsibilities

• Assess and reevaluate patient's status and adjust the plan of care with RN Case Manager.
• Provide focused physical assessments considering emotional and spiritual factors.
• Attend and support at time of death per physician order and regulations.
• Maintain communication with attending physician and RN Case Manager about changes.
• Document clinical records accurately and timely in the electronic system.
